To prepare for your visit at The Centers for Advanced Orthopaedics: Maryland Orthopedic Specialists Division, please complete the following patient forms:

• Demographic Paperwork
• Right to Privacy
• Consent to Treat a Minor
• Release of Medical Records

Please complete your health history information on our patient portal at cfaortho.ema.md.

• The default username is your email address.
• If you cannot access the portal, please contact our office for access to be turned on. The portal is new as of 1/25/2021.
• All office notes are automatically available on the portal once completed.
